Schalck, filled out for him at the Young Men's Christian Association of Chicago. Data points include strength and size/measurements for different parts of the boy, height and weight. A photo of a body builder at the foot is labeled with each body part. Includes notes at the bottom for "physical irregularities" and "deficient measurements" and "tests".
